Birth Certificate...name Format!!!

Hi Everyone,
I have decided to file for 485 and hence have started gathering documents for the same. I realised that my Birth Certificate only has the First names of my parents and myself. There is no Last Name mentioned anywhere on the Birth Certificate. Upon calling back home my parents informed me that this is the only format that is done back in my native place. in fact my father enquired from other sources and realised that some Birth Certs have the Last Name and some dont!!
Has anyone faced this situation??? Can I submit my Birth Certificate without the Last Names?? Or I need to get it re-written from my native place?
My Birth Cert has been translated into English and notarised back in India. Any comments would be highly appreciated!!! Thanks.

yeh u need to have have that in birth certificate. check with you lawyer might be you can do with affidavits.

Need Lastname

Undoubtebly you will need the last name if you are submitting the Birth Cert. You can get a new one with last name from your native place or maybe you can do with affidavits IF you can provide a non-availability certificate from a local govt. authority along with it. Normally in India it is signed and stamped by the secretary of the local panchayat or muncipal office. It is also a standard form - all panchayats should have them or they can always get them.
