Birth Certificate - NAC from whom?

Gurus,

I do not have a Birth Certificate issued and I would not be able to get it from India as my Birth was never registered when I born in 1969.

I would like to get NAC and 2 Affidavits from Parents and Relatives. I am confused from whom I should get the NAC. Should it be from only the local municipality office or it can be a from a Magistrate of the local jurisdiction.

Would appreciate your responses.
