I used the following format:
AFFIDAVIT OF BIRTH
AFFIDAVIT BY
Father's name and
Mother's name
We, father's name and mother's name, being duly sworn, do depose and make the following statements:
1. Our full and complete address is XXX
2. We are citizens of India.
3. I, father's name was born in place of birth, State, Country on Date Of Birth.
4. I, mother's name was born in place of birth, State, Country on Date Of Birth.
5. We married on date of marriage in (city name, state and country).
6. Your name was born on xxx at xx p.m. in CITY, STATE, India.
This affidavit is being submitted because there is no official record of birth concerning Your name.
We declare under penalty of perjury, that the foregoing is a true and correct statement.

Guys,
I have an issue with the birth certificates for me, my wife and my daughter. All of us were born in India and do not have our names on the birth certificate.
Will this be a problem when filing 485? If so do I have to get an affidavit and in what format?
Any help is greatly appreciated.
