Birth certificate issue

I have a question on how my affidavit (from my parents) should look like if
the names used in my birth certificate, my school records and my passport
are different. I have so far used a different first name from the one shown on
my birth certificate. Also I have given my last name in front of my first name
in my passport, as opposed to that in my school records. Below I
have given an example as to how my name differs in various documents.

For example, my first name is Ram and last name is Sharma.
Name in birth certificate: S. Raam
Name in passport: Sharma Ram
Name in 5 years school records: Ram .S

1) How should the affidavit from my parents look like ?
2) Can I get one affidavit from both my parents which states that my first
name is Ram and last name is Sharma, and that all the names as is available
in these documents refer to the same person ?
3) Is there any other legal document I need to submit in this case ?

The affidavit should mention your name the one in birth certificate S Raam. For ex this is to certify that SRaam also known as your other names was born onxxx. One line stating the fact that all three names are for one same individual should be included. You need to have documentation that all names belong to same person. I hope you have filled the other names in the other name column of the forms you have filled at the present time and previously while you applied for non immigrant visas.
