Birth Certificate Issue

Hello Everyone !

My Birth Certificate from India does not have my name. It just mentions my parents name , sex , date, and place of birth.

What should I do ?

Do you have your date of birth on any of your school marksheets?

If you do then you need one affidavit from either your Mom or Dad?

If you do not have that then you need 2 affidavit .. One from either Mom or Dad and second from a close relative who was 18+ years old when you were born.
one affidavit from Mom and one affidavit from Dad will NOT work.

format is simple ...

Name of person who is signing the affidavit (Mom or Dad or relative)
His/Her DOB / Place of Birth
His/Her spouse with relatiohip to you and to the perosn who is signing affidavit .

Date and place of marrige of person who is signing affidavit
and that he know that you were born on so and so date and such an such place.

Non Availability of Birth Certificate

My wife doesn't have birth certificate and its not possible to get one also. In this case, I heard that we've to produce affidavits from Mom/Dad and also a certificate from Goverment authority stating that Birth certificate is not available. Does any body know how to get this? Any format for this certificate?
