Birth Certificate Format

Hi,
I would like to know the format for Birth Certificate which we need to get on a stamped paper if we dont have the original birth certificate.

If anyone has the link or the content which needs to be typed in a stamped paper, it would be greatly appreciated.

My understanding is Father, Mother should give a statement in the stamped paper that needs to be notarized.

Apart from Father and Mom, someone from the boy/girl's side should also do it on a seperate stamped paper right?

Correct me if I am wrong?
