birth certificate for mother

My mother's brith certificate is not available. She is 55 yrs old.

I got a non-availability certificate from the local municipality office from her birth place and 1 affidavit from her older brother..

She doesnt have any more relatives and my father passed away few yrs back...


a) Do I need 2 affidavits along with the non-availability certificate ?
b) If I have only 1 affidavit available, where can I get the second affidavit from ?
