Birth Certificate doesn't show Last name

Hi,

I am sponsoring my parents to US. But on my Birth certificate, I don't have the Last name of me and neither of my parents and we only have the First and the middle names. For example, Ajay Singh and Simran Kaur. But on all the other documents of me and my parents (like passports, and my naturalization certificate) we do have the Last name. (example: Ajay Singh Sodhi)

So, do I need to submit an affidavit along with the Birth Certificate OR I can just submit my Birth Certificate as it is?

Please help.
