Birth certificate details for sponsoring GC..

Hello,

I am planning to sponsor GC for my parents and my question is regarding my Birth certificate document. I have a birth certificate available with my full name and my father's and mother's name.

1) But my father's full name/initial and Mother's full name/initials are missing.
Will this be a problem?
2) Also the place of birth just indicates the city in India and not the country (India).

3) Will I need an affidavit for providing these additional details (missing country, parents full name) and should that be notorized? Is there any format to be followed for this affidavit?

-- In my case also, only 1st name of parent was there. No problem.

2) Also the place of birth just indicates the city in India and not the country (India).
-- In my case also, Only city, no country. No problem.

-- Well, you have to submit your parents birth certificate too. In which they will have full name.

They can do affidavit by local court, which says that, what are their full names and you are their daughter/son (and other siblings, if you have any). Single affidavit will work, they both will sign.

Note: I am US Citizen, applied for parents(in Ahmedabad) in Aug 2009, their interview in Mumbai on 4th week of Jan 2010.
