Birth certificate and Non-avail letter

I have gone through number of threads but failed to find out what exactly the Non-Availability Letter from Municipality should mention. Can somebody please send me the format of that. My case is simple I dont have a birth certificate so what I understand is that, I need a 2 affidavits from parents and Non-avalibility certificate from the municipality.

Go to "Adjust of status(I-485)" and then click on "affidavit of birth".
