Birth Certificate Affidavit

Hi,

My Birth Certificate does not has my name on it. I believe you have to give a affidavit of support that states that I was child mentioned in the Certificate. Can anyone provide me with the format of this affidavit. I will appreciate any answers from all the Gurus.

Attached is a sample affidavit of support which i verified with my attorneys last week since i also needed to get the affidavit from my parents as their name is not written in full on my birth certificate....
