birth affidavit

I have non-availability certificate, since I dont have birth registered. Is it ok if i get birth affidavits from my mother and father? Or, do i have to get at least one affidavit from outside family?

It's fine to get it from father & mother.

Also cannon make sure that the affidavits issued include all the elements required in the standard affidavit.

For reference sake, you can see the attached sample affidavit that I have used.
