Answer to your question is very simple: YES!

My attorney advised me to get the "statement of continuation of future employment" from my employer. This letter can be exactly the same as that was sent with the I-485 \'adjustment of status\' application. I did exactly the same. I presented my employer with the old-letter and asked them to make a new one with current date and change few other details. In your case you might not have to even change the contents.

address from three people. Should it be signed by the
people in front of the notary or notary will notorize

I got the same letter this week. I am doing exactly what Beemboy and another guy advised me. The extra line which I have added (and I hope my company signs it) is that the employment will continue after the green card.

STRICTLY speaking - Upon confirming the document contents, the notary will advise the affidavit provider to take oath and sign the document. The Notary will then sign and stamp the document.

GENERALLY speaking - The document should be signed in notary\'s presence. The notary will request the affidavit provider to sign the document, and then signs and stamps the document.
