Be Cautious on Labor Substitutions

Lately INS has rejected the Labor Subs cases in DE, NY and NJ.

Why would labor substitution cases go to DOL? I thought that
I-140 could be filed directly.

I am missed that it is I- 140 at INS.

how would ins know about labor substitution? Is Labor filed for a particular person or for a position in general?

because on the labor certification , the name of the alien is given , from there they come to know, secondly, when the employer files for I-140 for the new person, he has to give the reasons for such change and then it is upto INS to approve those reasons or not.
