i have my original BC from munucipal corp from India but the registered name is different that the actual name, as my parents decided to change my name after registering my birth in 1972.
Get two affidavits from your parents testifying that you are teh same person as on BC and they changed your name at a later date.
Also include the standard information as required in these type of affidavits.
