1. My birth certificate has names of my biological parents and all of my other documents (passports) have the names of my adopted parents. What documents would be required for my case?
1a. If my biological parents are unavailable, what options do I have?
2. My birth certificate does not have my name on it. Further, the place of birth is filled with the hospital name or acronym and not the city name. What documents do I need for this case (Please remember that I am adopted)? If any affidavit is required from parents, should it be my biological or adopted parents?
3. My wife's birth certificate is re-issued during 2005 because she lost the birth certificate. However, her birth was registered within 5 days of her birth and it is mentioned in the birth certificate. Would this need any additional documents? If so, could you please list them?
4. My wife's birth certificate has her first name only. Further, the place of birth is filled with the hospital name or acronym and not the city name. What documents are required for this case?
