B2 visa rejected due to extension last trip

My mother's B2 visa got rejected at Chennai. The officer asked various questions and she answered them all. The contentious issue was her extension during last visit. The officer initially asked her to come back with her original extension papers (she only had approval copy the first day). Next day she went with all relevant documentation - Original Extension receipt/approval, copy of 539 application, copy of original I94, passport stamp that shows when she left etc. The documents clear show that she never was out of status and left before the expiry. But next day he rejected her saying there was no guarantee she will not extend her stay this time as well (she asked for 4 months in DS156). I am not sure why he even asked her to come back with relevant papers if he was going to reject her. Oficially the letter say 214 reason.

(Background - She came in 2001 with 6 month visa. After 4 month she applied for extension. Got the extension approved for another 6 months. She stayed for 5 more months (total 11months). So everything was legal and on time. She was not out of status and went with still 1 month left on her extension.)

Does this mean you should never extend while on a 6 month visa? I was wondering what others had experienced when visa was extended sucessfully and they applied the next time at consulate? Also, what are some of the valid arguments for extension that can be used for future interviews. (I had just used tourism to get extension approved in 2001)
