B1/B2 visa for Medical treatment - what are my chances

Hi,
I am a post doctoral fellow with a J1 visa. My parents came to visit me in April 2010 on a B1/B2 visa (issued in April 2001 and expiring on April 1st 2011). Unfortunately during the first month of their stay, my dad developed back pain, which progressively got worse. Finally, during an emergency room visit on May 3rd, 2010, he was diagnosed with a tumor in his vertebra, which needed immediate radiation therapy. He was later diagnosed with Advanced Stage IV Non Small Cell Lung Cancer. Since, his treatment options were not very promising, the doctors advised us to look into clinical trials. He was accepted for a clinical trial in NIH, Bethesda and is currently undergoing the clinical trial and the doctors say that the disease progression appears to be stabilized. However, in order to maintain this status, he needs to continue with the trial for about 18 months.
The following are the issues
1. His passport was supposed to expire in Nov 2010. After visit to Embassy and pleading his case, they issued a Tatkal passport valid till July 2011. We are currently trying to extend this passport.
2. Both my parents' extended I-94 is valid till APril 5th 2011. They need to know if it would be possible to return to India (Chennai) and apply for a new B1/B2 visa with a support letter from me and a detailed doctor's letter from NIH.
We also would like to know the probability of success in procuring a new B1/B2 visa.
My dad is retired and my mom is a house wife.
They have some property in India and some investments and bank balance.
Both their sons are in US (one on H1 visa and the other on J1 visa) for the past 4-5 years.

The chances for another B1-B2 are very slim given their situation.
Instead of another B1-B2 their chances to extend or change their status are much much better with detailed Doctor's recommendations in US.
