At PoE, after CP

I understand that non-immigrant visa holders submit I-94 (white form) at PoE. And permanent residents submit I-94W (green form) [or should I say, used to submit, as I read in another thread that the GC is swiped at ports of entry and exit, instead of I-94W procedure for PRs]

When immigrant visa holders enter US after CP, which form should they submit--I-94, I-94W, or some other form?

only the US Customs form ...

nothing else ...

The green I-94W is for citizens of those countries which have the visa waiver program with the US ... they use I-94W instead of I-94 ..
