asking for help with I-131 form

new comer here, so impressed by the informatives!
A AS7 Green Card holder from 2005, I am going to apply reenter permit. ask you guys to help me with some of my confusions.

1, how to fill the "Class of Adimsion"? or just leave it blank?

2, is there a item in the reenter permit showing category as7 or as8 or as6?

3, what I am really concerned is AS7 will appear in my reenter permit if I fill in AS7, which is the main reason I repply for the Reenter Permit.

2-No.

There is no status shown up on Reentry Permit.
