AR11 question

I am a 485 adjustee and recently moved to new address in state need to fill out AR-11, I used AP in last august and got a I-94 valid till Aug 25,2003

Now what should I fillup at
1.

I AM IN THE UNITED STATES AS:
Visitor , Permanent Resident
Student , Other . . . . . . . (Specify)

Is "adjustee" right word
2.
DATE OF ENTRY INTO US

Is it the last entry in US or very first entry in US

3.
IF NOT A PERMANENT DATE OF ENTRY INTO U.S.
RESIDENT, MY STAY IN
THE U.S. EXPIRES ON:
(Date)

what do write here: till my application is processed

My understanding is that its more important to call up VSC to update your new address, than split hair on the entries in this form. I believe these forms which goes to Kentucky take their own sweet time to reach the desk of IIO. Perhap by then you would be approved
