AR11 Question

friends,
I have couple of questions on AR11 Address change. Please help me understand.
1. In the AR11 form, the coloum that says, "Port of Entry into US" what to write in that. Is that the information of first time entry point in USA or the last time when you entered in USA.

2. Is there any supporting document required for filing AR11
3. where to send this form, to the address mentioned at the bottom of the AR11 form or to the Texas Service Center where your I-485 is being adjusted.

I beileve the Last Port of Entry.

I believe No. (this is not a petition, its simple update address request.)
Make copy for ur record and send this form using USPS Certified with return reciept required.

The Address written on Form, TSC has to do nothing with it.
