AR-11 + late submission, date/port of entry questions.

Moved few weeks ago and discovered that I've missed the 10 day cut off for submitting AR-11! Is this a big problem?????

I'm a permanent resident and don't know what to answer for the fields which require me to enter my "port of entry into the U.S." and "date of entry..." Are these the date and service center where I was granted residency? Or I don't need to fill them out since I'm a PR? Or was it my last entry into the US?

The date is date of your last entry into US, whether you were an LPR at that time or not. Your date of LPR status is not important. AR-11 needs to be filled out by everyone except US citizens.
