ar-11 form filling problem

I am working in H-1 Visa. I recently got EAD approved. I also got finger print notice. But now i changed my address.

The immigration guys asked me to fill the ar-11 form for me and my wife.

I have four questions in that form.

Question 1.

I AM IN THE UNITED STATES AS :
a. Vistor
b. Permanent Resident
c. Student
d. Other ---- H1-B Non Immigrant ??

I guess its "d" but i dont know other things.

Question 2.
COPY NUMBER FROM ALIEN CARD
?????

Question 3.
PORT OF ENTRY INTO U.S.
DATE OF ENTRY INTO U.S.
Is this the last entry into US. ???

Question 4.
IF NOT A PERMANENT RESIDENT, MY STAY IN THE U.S. EXPIRES ON. (DATE)
Is this date is the H1-B expiration date ????

Please help me!!!!!!!!!

A# (u can find in ur EAD or in any receipt notices

3)
Yes, ur last entry (place and date)

4)
yes, ur H1B expiration date
 
FYI : To change your address with USCIS on pending petitions(i-140/485), you will need to call Service center and change address on each application.

as H1-B Non immigrant (485-AOS pending).

AOS: adjustment of status.

AR-11 form should be filed by Attorny or applicant?

Hi, Should AR-11 form be filed by myself or by Attorney? Does each applicant requie one AR-11 form ? In the form "Employer column" do we need to mention the company name which application got filed or present working compnay? When should we call BCIS for change of address? Is it after filing AR-11 application ? or without filing ?

Please help in finding all the above details.

It's a simple form and you need to fill in ..., not the attorney.

Each person in your family need a separate application. Read instructions from the AR11 form for more details.

You may give your present company's address.

Once you post the forms by certified mail (Just to track that it reaches), you can call them.

You need to do three following things for address change.

1. Send AR 11 forms.
2. Send copies of AR 11 with a covering letter in whcih details of applications pending with them by giving EAC # A# Name etc... (Search forum for details)
3. Talk to USCIS representative by calling 1-800-375-LATE. (Make sure if you are married , your wife should be there with you when you call or she has to call them separately)

You will receive address change confirmation letter from USCIS after one month. Don't be surprised even if you still get letters/notices to your old address ..

That's it..
