ar-11 filing - used AP recently for Canada trip ...

I used my Advance Parole to a trip to India a few months ago and last month I went to Canada (driving from Seattle to Vancouver, Canada) and used Advance Parole to come back (driving again) into the US on Nov 1, 2005.

I moved to a new apt this week and am filling the AR-11. It asks for last date of entry into the US and the port of entry.

My question is:
Should I use the last date of entry as Nov 1 2005 and POE as Seattle?

Yes you put in your last entry date.(Nov 1) And where is it you crossed the border. Seattle is not the border crossing. Did you come through the Blaine, WA border crossing, etc. I did it in Manitoba and I put in the name of the border crossing itself. Which for me was Pembina, ND.

Thanks for your replies. I was somewhat naive in thinking it's Seattle, WA but after your posts I checked the POE info and found that it's:
Blaine-Peace Arch POE, Interstate 5 and International Border, Blaine, Washington (WA)
