Approved I-140 and pending I-485

How INS works if I-485 is pending and in the meantime I-140 is approved? Do the attorney has to inform INS about approval of I-140 or INS itself figures out from their system that I-140 has been approved for such applicant?

Your attorney donot need to do anything. It is the same file. Once 140 is approved it will update the 485 as well.
