Short Story:
My home address changed 0 times

No of times laid off = 1 (joined same company :) )

No of lawyers used = 2
No of times State Tax ID of company changed = 2
Finger printing 2 times
EAD 2 times
AP 2 times

My company management changed 3 times
My company name changed 3 times
My company place changed 3 times

No of times GC approved = 1 :)
