Applying GC for spouse

I recently became a citizen and would like to apply GC for my spouse who is already in US. She is in F1 student status and currently has OPT. My ques. is:

1. Can I just file form I-130 & I-485? and leave out I-765 since she already has OPT.
2. What do you mean by filing I-130 & I-485 concurrently? Do I file these applications at different service centers?
3. Does she need medical exam report?

1.- Yes. I would go ahead and file I-765 just to give her more freedom.
2.- Filing them concurrently means at the same time. File them to the Chicago LockBox.
3.- Yes, she needs to find an USCIS civil surgeon and have him fill out I-693.

Note: Try to submit these before July 30th. As new filing fees kick him that day.
