Applying for EAD (SFO)

I am planning to apply for my EAD (myself)
1. I found the 765 form in the website
2. i do not need a signature card(right?)
3. the fees enclosed will be $120/-

All i have to do is fill in the form, enclose the check and the 485 receipt

Pls confirm...Jaxen...or anybody who has successfully done this themselves..
