AP Renewal Question

I found the revised I131 form in immihelp.com.

1) what should i put in Part1-C
"Class of Admission"
I was in H1 and now i changed job using EAD.

2) Can any one tell me the TSC address for sending
the AP application? form instructions shows only nebraska

3) Form Instructions says check payable to "US Citizenship and Immigration Services" and the immihelp.com says
"USCIS-TSC". which one is correct?

Have you figured out what is class of admission? I got same problem. If you got the answer, would you please let me share the answer. I came as F1. Now I am just I-485 pending with expired H1. Should I fill F1 in the space?
 
Talked to IIO

Called the 800# for this question. The Cust. Rep did not
have the new version of form so he transferred me to an
immigration officer.

IIO told me that, i should put the manner of last entry.
I told him that i entered in H1B and recently i started using
EAD and i am not in H1B anymore. He said it, doesn't matter
still i need to put the class that i was in during port of entry.
