AP question

Hello friends,

My visa is expired and need to use the AP.
I need clarifications to fill I-94 form while returning.

1. City where visa was issued.

2. Date issued.

I left blank those fields

Put N.A (not applicable) to those fields. When you enter the country display the AP to the immigration officer and he/she will guide you to the secondary inspection, where the officers are a lot nicer and know more of the entire process.
