AP Filing questions

I am planning to file AP for myself and my spouse @VSC.

1. Are checks ($110) payable to USCIS now?

2. Can both applications go in one package with ONE cover letter?

3. Besides the fees, I-131 aplication and 2 photos should the following documents suffice:

Copy of I-485 receipt notice
Copy of current and previous AP(s)
Copy of I-140 approval notice
Bio. Page of passport and copy of I-94
Copy of Marriage Certificate (spouse only)

I am also noticing postings regarding RFE for APs.
What are they for? Just to be on the safe side for filing AP @VSC.

There is a new form available for AP filing.
In Part 1 of the form, 'Class of Admission' is requested.

What is this 'Class of Admission'?
