redhat,
I havn't had the need to call the 1-800 Customer Service number yet but lot of people on this board have complained about somewhat ignorant people answering the phone. So I would be careful with any response from them. May be "US Treasury" works but I don't know.
As I said, I made the checks payable to "Bureau of Citizenship and Immigration Services" when I sent my applications early July 2003. Since then the checks have been cashed and I have gotten the I-797 Receipt Notices.
Documents for I-131 (AP)
------------------------
1. I-485 Receipt Notice
2. Attachment stating why I need AP
3. 2 photographs
Documents for I765 (EAD)

1. I-485 Receipt Notice
2. Copy of I-94 (front and back)
3. Copy of previous EAD (front and back)

If you download and read the instructions for the two forms from the BCIS website, these are the documents they ask for.
Good Luck,
