AP application returned

Hi,
This is my third AP application.
My application was returned by BCIS indicating missing page 3.

Does anyone have any idea what this 'missing page 3' mean.

My last application ( filed self ) got approved with same set
of documents.

I am the primary applicant.

I attached the following docs :

1) 485 receipt,
2) I-140 approval
3) last AP approval copy
4) I-131
5) cheque for 110
6) photos (2)
7) driver license...

I appreciate any answer.
