AOS Processing Fee

After sending the DSP-122 to KCC I received a letter back from them stating a few things including this:

You must also be sure to follow the instructions on the attached sheet concerning payment of the Diversity processing fee. This fee must be paid prior to USCIS (or, if applicable, the immigration court) taking action on your adjustment application.

Problem was that there was no attached sheet.

Does anybody know what form number should have been attached and if I can download it?

Hi there,

The letter you've received is an acknowledgement letter.
Don't worry about the missing attached sheet, I called them too about that.
All you have to do is to pay your DV fee to the Department of State using the slip provided in your NL. Wait for a recipt (5-10 days), make a copy and keep the original in a safe place since it's one of the most valuable pieces of paper needed in order to get a GC!
After that you will pay Homeland Security the appropriate fee when applying for AOS.
