AOS forms required petitioner and sponsor

I have rung the immigration customer service line so many times, and they all give me different answers so now i'm hoping someone here can help me.
I'm applying for adjustment of status I need to know the following:

a) what forms do I need
b) what forms does my husband need
c) what forms does my joint sponsor need (husband does not meet income requirements)

AND what shots did you have for your medical? I realise everybody is diff, but I"m trying to get an idea of the 'common' ones, the only doc here wants to charge me $260 for my shots!!! SO I'm trying to get them done at my main doc where she will charge me $71

If the I-130 has alrady been filed for you by your husband, you need the I-485, I-693 (Medical cannot be done by any other doctor otherthan on the list that uscis.gov has - I paid 255 for a medical exam and MMR and Td shots (no varicella), PLUS 165 for a Blood Hiv and Syphillis test and XRay) and I-765 and I-131 if necessary, and husband's I-864. Additional sponsors as specified in the I-864 instructions will have to fill the forms as well (not sure if I-864/A).
