**Please make sure all COPIES are Clear and Legible


1.Original I-140 Approval Notice /Receipt Notice (Legal Dept. will have this)

2.*Copy of Birth Certificate of Employee and ALL Dependent Family Members
(Please provide translations if not in English)
** If NO birth certificate is available then a “Non-Availability Certificate” MUST BE
SUBMITTED with Affidavits of Birth, Executed by two immediate family
members who were 5 years of age or older at the time the birth took place—
(Please see sample below)

*** If a birth certificate was recently issued, then it MUST BE SUBMITTED along with Affidavits of Birth, Executed by two immediate family members who were 5 years of age or older at the time the birth took place—(Please see attached sample)

**** Please also include U.S. Citizen Children’s Birth Certificate(s)

3. Copy of Marriage Certificate
(Please provide translations if not in English)

4. Copy of all Divorce Decrees
(Please provide translations if not in English)

5. U.S. Federal Tax Returns - IRS Form 1040 for the last THREE (3) years
*(Please do not send W-2’s unless requested)

6. 3 Months of most recent Paystubs

7. 6 photos of employee and each dependent

8. Form I-693 Medical Examination – by U.S. Civil Surgeon
(Please call 1-800-375-5283 for a list of registered U.S. Civil Surgeons in your area)

9. Copy of ALL pages (including blank pages) of Current and Expired passports for employee and dependents
**Please make COLOR copies of the front page of your passport(s) and visa page(s)

10. Copies of ALL I-94 cards, I-797 Approvals (H-1B & H-4s), I-20s, and EAD cards.

11. COLOR Copy (front and back) of State issued Drivers’ License and/or State Identification Card

I was thinking about starting a thread on this, here you are with all the details. Thanks a lot for the detailed list.

I have been toying with the idea of filing 485 by myself. I know that I need to submit employment letter along with this list for the primary applicant. Is there anything else I need to consider if I file 485 myself?

Would the courtesy 140 copy that was sent to employer is enough, or do we need the one that was sent to lawyer?

Is there a certain minimum validity period for the passport? Mine is expiring in Nov 2007, and am in the process of getting it renewed.

I have copies of all H1b's and H4's, and the recent I-94's. But I don't have copies of some of the I-94's that got issued 6 or 7 years ago, which I had returned during travel.

Should we somehow try to get copies of all the I-94's?

Couple of people whom I know and who filed themselves, got RFEs and all of them stucked in the Name Check for several years.

There is a rumour that if case is filed by lawyer, it reduces chances of long name/background check.

AOS Checklist ... address for correspondence

Folks!

I am going to file for i-485 in few weeks. And a month after that I am moving to new apartment. Both the old and new places are in same state (Michigan). So it is merely home location change. But I don't have this new apartment address finalized yet or in hand.

Now I have read in some of the threads and posts that changing the address at USCIS is not straight forward and simple. And USCIS continues to send correspondence ( if any) at address mentioned originally in the application ( which in this case would be my old apartment address).

My question to all of you is; Can I use the address of my friend who lives in another state (Illinios)?

If you check the G-28 check box in I-485 .. then all the correspondence goes to your attorneys office and not your home address.

I know some guys who have had used there friends address -- for correspondence ... they have not got into any trouble yet .. there 485 are still pending.

But my argument is that, if you do anyones background check by paying $7.95.. one can get all the addresses one every lived and all other information. During the name check and background check that is done during 485 .. one MIGHT experience some delay .. [well these are my thoughts ]
 
I dont have all the I-94 copies.Is that ok ? I never knew that i was supposed to keep a copy of all the I-94's.Let kenow

Do we have to submit the job offer letter as well with these applications.
What about the I-94 card? Copies of All I-94 cards to be submitted or only the recent one.?
 
Thanks for this information. I have couple of questions:
1) Can we submit copies of birth/marriage certificate? If yes, then do they need to be notarized? If yes, then does it need to be notarized in US or can be done in India
2) Is it fine if the primary applicant (my wife) and me have medical checkups from two different UCSIS authorized doctors- one in CA and one in AZ because we work in different states. Our lawyer said that we need to show that my permanent address is in CA since thats where the primary applicant is. Is that required? But then, I will also be sending my H1B which shows that I work in AZ.
