Anyone had I-539 approved and there was no date on new I-94?

My mother had her petition for EOS approved but the new I-94 has no date of validity.
Is that possble or it's a mistake of INS?
It only has file date and notice date.

I am sorry I don't have answer to your query, but if you don't mind, can you give me some details regarding what reason for extension did you give? When did you apply? where? Did you get approval BEFORE I-94 expiry, etc? Thanks so much !
