Does it create any problem at the time of visa stamping if H1's LCA document has yearly salary mentioned as 42k per year though employee gets paid 120K per year ?
Could this excess amount be paid as overtime and bonus ?

It is not good to have a salary figure mentioned in LCA and the individual actually ends up getting paid 3 times more. This can have any kinds of problems -- it could have been an incorrectly filed LCA. With the LCA being the subject of your H-1B petition, it can easily have an effect on your H-1B as well.

Thanks for your response. Would it be same if the person's filed hourly pay rate in LCA is as minimum as around 20 dollars per hour and if person gets paid as 60 dollars per hour?

Does he need to carry any additional documents to support this additional paid rate?

Again, if the LCA salary figure does not match with the actualy salary that the individual is getting paid, whether it is hourly or fortnightly or monthly or yearly, it does not matter, bottomline is that the Job based on the LCA is not legal.
