Any body done eFiling of AP please help

I had done eFiling for my Wife and Kid. I have some doubts/clarifications.
1) Do we need to set up appointment with Local Service center for Photos
or we need to mail the Photos along with supported documents.
2) What are the required supporting documnets?
3) While eFiling some where it is asking to "explain how you qualify for an
AP document and what circumstances warrant issuance of AP". What do
we need send or write for it?

Please help me on above issues.

I efiled AP on 02/18 for myself and my wife and mailed all the docs on 02/19. This is the list we sent. I dont know if its all conclusive. Also search the forums for the eligibility letter as I dont have its format right now with me:

 2 passport size photographs
 Confirmation receipt
 Letter of eligibility for Advance Parole
 Copy of I-485 petition receipt ( AOS Application pending with Vermont Service Center
 Copy of Current Advance Parole document
 Copy of EAD (front and back)
 Copy of current H1B
 Copy of Driver’s license ( coz they said needed biographic info)
