Another question about the PCC Application

One of the requirements on the PCC App. is "Personal Particulars Proforma". The consulate website says one needs to submit: 3 copies and 3 photographs.

Don't call me silly!!! :confused: Isn't it just 3 copies of the proforma with the pasted photos.

Or is it 3 extra photographs in addition to already pasted ones?
 
I don't think that the form in triplicate is reqd. At least not at Houston. If this is SFO, this was discussed in the forum sometime back.
