After one Yr of J1 residency

Hi
I am on J1 residency program and in 1st yr.I am planning to move to another speciality next yr. In case I dont get the position and the current program director did not extend the contract for the next yr, I will be out of status.
My question is that, Is there any chance that I can apply for another visa with out going to home country?
Are there any other loops that can make me to stay in USA?
Is there any chance that I can convert to J 1 clinical after this?
Pls guys let me know about this issue before I do something stupid thing.....

You should be able to convert to a visitors (B) or even an student (F1) with a valid I-20 once you are admitted for a degree,etc.
