Is filling out the I-134 form necessary for H4 visa application? If so, do I need to sign it at a notary office?

I don't remember taking I-134 for my dependants on H-4. I used that on 2 occasions - while sponsoring someone on visit visa and while appyling I-485 for dependants.
 
I had the same thought as you, but I found some people in this forum who included I-134 form along with their H4 application. That's why I am not sure if I should fill it out and go to notary office for it or just skip that.
