how to get an affidavit of identification ???

HOW CAN GET AN AFFIDAVIT OF IDENTIFICATION?? WHAT ARE THE REQUIERMENTS AND HOW HARD IS TO GET?????

ok can someone tell me what is the affidavit and.. if i can get being and b1 and b2 visitor.. and also .. what are the ways to get one?? thankz!!
