You dont't need that form as long as you don't have direct demands from the consular representative that you have to bring one. Normaly it is good to have it, but science you time is so close you could try to go by like i did.
My friend from USA send me a mail with this content, i made a print copy of it and show it to the consular representative.

To whom it may concern,
I am writing this letter on behalf of (first-last name of aplicant) and his family and asking that it be received by you as a formal letter of invitation by myself, for them to stay in United States for indefinite period of time.
It is my understanding that for them to be given a Green Card, I must extend a formal invitation.
As a permanent resident in United States they will stay in my home at (address of your sponsor) as long as needed.
If there is any additional needed to complete this process, please don't hesitate to call me at my number (phone number of sponsor)

This helped me at the embassy. But if they still insist on 134 form, you will be given additional days to bring that document so it is not a big deal anyway.
Just be positive and explain to them the situation.
Dont get mixed with the dates:
Wednesday = 3. March
Tuesday = 2. March
