affidavit for CP

Do anyone have experience in submitting a non-availability certificate with affidavits as an alternative for birth certificate. I know a lot of ppl. who have done this for I-485. But does anyone have exp. in providing the same for their CP??
 
has anybody used affidavit from relatives instead of parents..

and also notarized 134 from city other than where your US permanent addr is mentioned in DS Part-I
