Affidavit for Birth Certificate

Hi friends,
Please help me with a format of Affidavit for Birth Certificate.
Should this be executed on a stamp paper? If so, is a Rs.10 stamp paper OK?

Is it required that the father and mother execute the affidavit separately or can it be jointly?

Check the thread 'Birth NAC and Birth Affidavit-Templates ' above.

Better to take two affidavits, one from father and one from mother.
If necessary it is OK to take these affidavits from some elder relatives like
uncles, aunts, older siblings and so on.

Take them on Rs.20 stamp paper.
