do you have to inform immigration on your address change after you get a green card?
If yes what is the form and where do you send the information to?

You need to still fill in AR11 form avaialble on the USCIS website unless you are a US citizen or have a G/A Visa status. The address to send the form is mentioned in the AR11 form (somewhere in KY if i remember).
 
I think you have to use form AR 11 and send this to Homeland security and a copy of it to USCIS.
