additional training during J1 waiver

i would like to do a part time fellowship and/or psychoanalytic training while i do the J1 waiver.
if all patient care is limited to the J1 waiver site during the 40hours/week, am i allowed to train in something beyond that?
would i require an additional H1B to do this ?
the training may offer a stipend.

my attorney feels that it is a violation since the laws state" full time employment at the J1 waiver site" and doing anything else implies it isn't full time.
i am wondering if people have a different opinion/experience about this.

You should be all right as long as you do the 40 hours weekly. That is all that is required.

You need a separate visa approval though.
