AC21 Success: Job description Database

I know there is a sticky thread for "AC21 approved."

Can we start another thread with job description details as well for those with approved AC21?

Basically if anyone used AC21 successfully, can you post:

1) original job description in LC and ONET/SOC code and
2) job description and ONET/SOC code in the new job?

Of course, if there is anything "personal" in such information please remove that.
This kind of database would be of enormous help to those who are looking for information...
